This happens when you talk about the effects of: over polishing, incorrect feed rate, worn drill size, being out of centre, having the wrong surface finish, material being too hard, plating being shadowed, radius being too tight, pilots breaking in scrap, swarf building in taps, grinding wheel needing to be re-dressed, stripper plate spring broken, tool not indexing, etc, etc (add your own here for me to add)
